Dissertation help needed!!!

I am in the throes of data gathering for my dissertation and need responses to a ranking survey.

 

It is literally as simple as 1-2-3, and sending me your e-mail address...and name.  It is a ranking survey and asks you to rank your top 3 preferred activities and send those choices back to me.
