Beginning coding class survey?

Hello!! I'm teaching a beginning coding class to high schoolers this semester. (My previous experience teaching coding has been to elementary school, so I need to up my maturity a little bit!)  ;)

Does anyone either have a great beginning survey to gauge kids' background and experience in coding, or can you suggest individual questions for the Google form I'm creating?

So far I have Q's about their general comfort level w/ computers, what kinds of devices they're used to using in their spare time, and which coding programs or apps they may already be familiar with (Code Academy, Code.org, Scratch, etc.).
