QUESTION: Differentiation for Transition Assessments?

I am trying to create a differentiated project or assessment for special education freshmen/sophomore students.  The title of the project is "College Readiness/Career Exploration."  Students will investigate learning styles, individual strengths/weaknesses."  The data will be used to guide educational job and career planning.

 

After the completion of the project/assessment, I want students to have a better understanding of their own personal strengths/weaknesses, obtain career ideas, and college information relating to careers.

My footprints of student learning would include:  pre/post tests on individual strengths & weaknesses, college readiness...possibly keep track of college information using Google Docs as a portfolio?
