Yes, if you have the book (and it sounds like you do) on page vii - at the end of the table of contents you can see the access code. Hysterically, I went to Amazon to check where it is and - well - you can see it in the preview. Soooo ... check it out!

The book is coming! We have done some revisions and there have been some unforeseen medical issues.  We will keep you posted.  We are almost there.

I've been checking on Amazon and B&N for the book every week since the beginning of January.  Any idea of a publication date?  Or am I looking in the wrong places?
